The swelling progressed gradually in size to involve the whole thigh. He has no previous treatment for that, no family of similar condition or local trauma. He has no other medical condition and was not on long-term medication.<\/p>\n<figure id=\"attachment_642\" aria-describedby=\"caption-attachment-642\" style=\"width: 960px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-642 size-full\" src=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_1.jpg\" alt=\"\" width=\"960\" height=\"1280\" srcset=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_1.jpg 960w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_1-225x300.jpg 225w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_1-768x1024.jpg 768w\" sizes=\"auto, (max-width: 960px) 100vw, 960px\" \/><figcaption id=\"caption-attachment-642\" class=\"wp-caption-text\">Fig 1: The thigh lesion at the presentation<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p style=\"text-align: justify;\">On general systemic examinations, no abnormality was detected. Local examination revealed a massive right thigh mass around 40 by 30 cm,\u00a0 extending from the right knee to the groin area. The right limb was externally rotated, with obvious asymmetry between the two lower limbs. There were multiple active sinuses discharging sero-bloody fluid. The mass was non-tender, of normal temperature, firm in consistency with no local hyperhidrosis<\/p>\n<figure id=\"attachment_641\" aria-describedby=\"caption-attachment-641\" style=\"width: 1430px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-641 size-full\" src=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2.jpg\" alt=\"\" width=\"1430\" height=\"1429\" srcset=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2.jpg 1430w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2-300x300.jpg 300w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2-1024x1024.jpg 1024w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2-150x150.jpg 150w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2023\/01\/Fig_2-768x767.jpg 768w\" sizes=\"auto, (max-width: 1430px) 100vw, 1430px\" \/><figcaption id=\"caption-attachment-641\" class=\"wp-caption-text\">Fig 2: The lesion after six-week treatment<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Laboratory and imaging investigations confirmed the diagnosis of Streptomyces somaliensis caused actinomycetoma. He was started on\u00a0 Amoxicillin\/clavulanic acid one gram twice a day, Co-trimoxazole 960mg twice daily, and folic acid 0.5mg once daily.
