{"id":4109,"date":"2024-03-13T15:42:16","date_gmt":"2024-03-13T15:42:16","guid":{"rendered":"https:\/\/mycetoma.edu.sd\/?p=4109"},"modified":"2024-03-14T10:43:45","modified_gmt":"2024-03-14T10:43:45","slug":"mycetoma-a-long-journey-out-of-the-shadows","status":"publish","type":"post","link":"https:\/\/mycetoma.edu.sd\/?p=4109","title":{"rendered":"Mycetoma: a long journey out of the shadows"},"content":{"rendered":"<p class=\"card-date\" style=\"text-align: justify;\"><strong style=\"text-transform: initial;\">Written by Professor Roderick Hay, Dr Kingsley Bampoe Asiedu and Professor Ahmed Fahal (based on the <a href=\"https:\/\/academic.oup.com\/trstmh\/article\/115\/4\/281\/6033507\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">editorial of the same name<\/a>\u00a0in\u00a0<em>Transactions<\/em>)-\u00a0 14 Apr 2021<\/strong><\/p>\n<div class=\"view view-sections view-id-sections view-display-id-main view-dom-id-6abe9969c3739bcc7ad4b86e82630115 jquery-once-1-processed refresh-processed\">\n<div class=\"view-content view-content-main\">\n<div class=\"section-inner\">\n<div class=\"card-body\">\n<p>Mycetoma is an important disease that affects the poorest of communities, and which has, for a long time, received little scientific or clinical attention.<\/p>\n<p>&nbsp;<\/p>\n<\/div>\n<\/div>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-4110 alignnone\" src=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2024\/03\/Fig-2B-resized.jpg\" alt=\"\" width=\"704\" height=\"469\" srcset=\"https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2024\/03\/Fig-2B-resized.jpg 480w, https:\/\/mycetoma.edu.sd\/wp-content\/uploads\/2024\/03\/Fig-2B-resized-300x200.jpg 300w\" sizes=\"auto, (max-width: 704px) 100vw, 704px\" \/><\/p>\n<div class=\"section-inner\">\n<div class=\"card-title\">\n<h4 class=\"field-content\" style=\"text-align: justify;\">A recipe for neglect<\/h4>\n<\/div>\n<div class=\"card-original\" style=\"text-align: justify;\">\n<p>Mycetoma Research Centre in Sudan<\/p>\n<\/div>\n<div class=\"card-body\" style=\"text-align: justify;\">\n<p>In part, this has been due to the fact that it is only prevalent in certain countries or in specific regions of countries; also, its progression is slow, it is not painful and managing the disease does not form a major part of the training of frontline health workers.<\/p>\n<p>Taken together, these observations provide a recipe that ensures late presentation of cases, inadequate recognition by health planners and continuing patient disability with consequent stigma, mental health and rejection in local communities.<\/p>\n<\/div>\n<\/div>\n<div class=\"section-inner\" style=\"text-align: justify;\">\n<div class=\"card-title\">\n<h4 class=\"field-content\">Khartoum Call for Action<\/h4>\n<\/div>\n<div class=\"card-body\">\n<p>However, in 2016, in response to these major concerns,\u00a0<a href=\"https:\/\/dndi.org\/press-releases\/2016\/mycetoma-who-ntd-list-response\/\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">mycetoma was formally recognised as a neglected tropical disease (NTD) by the WHO<\/a>.<\/p>\n<p>The subsequent\u00a0<a href=\"https:\/\/www.who.int\/neglected_diseases\/news\/Mycetoma-Global-conference-calls-for-action\/en\/\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">Khartoum Call for Action on Mycetoma of 2019<\/a>, drafted by experts in the field, set out a clear challenge to promote global recognition and consensus, research, new interventions and engagement of governments, non-governmental organisations and funders.<\/p>\n<p>This challenge has now been taken up by many, but there is still much more to be done.<\/p>\n<\/div>\n<\/div>\n<div class=\"section-inner\" style=\"text-align: justify;\">\n<div class=\"card-title\">\n<h4 class=\"field-content\">A special issue on mycetoma<\/h4>\n<\/div>\n<div class=\"card-body\">\n<p>This is why we are thrilled to be\u00a0<a href=\"https:\/\/academic.oup.com\/trstmh\/issue\/115\/4\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">publishing a\u00a0special issue of\u00a0<em>Transactions<\/em><\/a>\u00a0which focuses on mycetoma and brings together some of the leading experts in the field, including Wendy van de Sande, Mustafa Elnour Bahar, Elsheikh Mahgoub, Nathalie Strub-Wourgaft, Alexandro Bonifaz and RSTMH President, Professor Gail Davey.<\/p>\n<p>The special issue touches on a range of topics, including the first and sole comparative clinical trial of a new medication for mycetoma that is only now underway in Sudan. This shows how current disease management guidelines have been built on a foundation of anecdotal treatment experience and consensus opinion.<\/p>\n<p>The resources required to assess drug efficacy accurately have not been deployed until now for the alleviation of mycetoma. Inevitably, the imperative for new drug discovery in this disease has been muted, partly because, in a market-driven economy, mycetoma does not appear on the therapeutic needs radar.<\/p>\n<\/div>\n<\/div>\n<div class=\"section-inner\" style=\"text-align: justify;\">\n<div class=\"card-title\">\n<h4 class=\"field-content\">Funding and innovation needed urgently<\/h4>\n<\/div>\n<div class=\"card-body\">\n<p>So new funding and innovative approaches to drug development are urgently needed. Yet it would be a mistake to think of mycetoma as an isolated challenge, as new strategies that might unlock the door for mycetoma treatment may also produce benefits in other infectious diseases.<\/p>\n<p>From studies published in this issue, for example\u00a0<a href=\"https:\/\/academic.oup.com\/trstmh\/article\/115\/4\/324\/6104319\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">\u201cMelanin production in coelomycetous agents of black grain eumycetoma\u201d<\/a>, we are beginning to recognise that the grain, the defining parasitic structure seen in all mycetomas, shares many of the adaptations seen in a complex microbial biofilm.<\/p>\n<p>In eumycetomas, those caused by fungi, grains also contain elements such as grossly thickened cell walls, containing chitin and glucan, which might provide suitable antifungal targets in other contexts; the grain is also often permeated by melanin, another potential drug target.<\/p>\n<p>Similarly, in finding better treatments for actinomycetomas, those caused by filamentous bacteria, there are potential benefits for other actinomycete infections such as systemic nocardiosis.<\/p>\n<p>At present, mycetoma patients frequently resort to the only source of treatment available to them, traditional medicine, further delaying access to diagnosis and assessment.<\/p>\n<\/div>\n<\/div>\n<div class=\"section-inner\" style=\"text-align: justify;\">\n<div class=\"card-title\">\n<h4 class=\"field-content\">The challenge of diagnosis<\/h4>\n<\/div>\n<div class=\"card-body\">\n<p>Laboratory diagnosis is another issue highlighted in the special issue, for example in the article\u00a0<a href=\"https:\/\/academic.oup.com\/trstmh\/article-abstract\/115\/4\/355\/6101239?redirectedFrom=fulltext\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">\u201cActinomycetoma laboratory-based diagnosis: a mini-review\u201d<\/a>. At present, provision of simple diagnostic assessments is limited throughout many endemic areas. Again, this is due to a mixture of lack of training and facilities.<\/p>\n<p>But as mycetomas are caused by more than 70 different species of fungi or bacteria, accurate identification of individual causes is a key step in prevention and treatment.<\/p>\n<p>New organisms are being recognised all the time and providing accurate identification through their molecular structure is increasingly important in this field. Solving this deficit involves an imaginative combination of training, new diagnostic tests and the recognition of reference centres to support new initiatives in laboratory diagnosis.<\/p>\n<\/div>\n<\/div>\n<div class=\"section-inner\" style=\"text-align: justify;\">\n<div class=\"card-title\">\n<h4 class=\"field-content\">Early detection<\/h4>\n<\/div>\n<div class=\"card-body\">\n<p>Early detection of cases is another key theme of this issue and features in\u00a0<a href=\"https:\/\/academic.oup.com\/trstmh\/article-abstract\/115\/4\/383\/6105777?redirectedFrom=fulltext\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">\u201cMycetoma and the Community Dermatology Program, Mexico\u201d<\/a>. This is equally critical to the delivery of effective control of mycetoma, a feature now highlighted in the\u00a0<a href=\"https:\/\/rstmh.org\/news-blog\/news\/a-landmark-week-for-ntds-as-who-road-map-launches\" rel=\"nofollow noopener\" target=\"_blank\">WHO NTDs roadmap milestones<\/a>\u00a0to meet sustainable development goals by 2030.<\/p>\n<p>These include developing rapid diagnostic tests and effective treatment, as well as establishing surveillance for case detection and reporting within an overall health framework to ensure access to affordable diagnosis and treatment.<\/p>\n<p>This depends on better methods of detection clinically, in the laboratory or by imaging, deployable at frontline health centres or in accessible hospitals or laboratories.<\/p>\n<p>Delivery of this objective involves stepping up the training of health workers to facilitate recognition of this disease, engaging with local populations and creating specific integrated programmes at both national and local levels, key objectives in the plans for the detection and management of NTDs of the skin (or skin NTDs), of which mycetoma is an important member.<\/p>\n<\/div>\n<\/div>\n<div class=\"views-row views-row-8 views-row-even views-row-last section-row palette-default \">\n<div class=\"section-inner\">\n<h2 class=\"field-content\" style=\"text-align: justify;\">Holistic management approach<\/h2>\n<div class=\"card-body\">\n<p style=\"text-align: justify;\">There is an essential need to adopt a holistic management approach to mycetoma patients that encompasses medical, health, socioeconomic and environmental support for patients, families, community and the health authorities in the affected endemic regions.<\/p>\n<p style=\"text-align: justify;\">The goal of reducing the use of, and eliminating the need for, amputation is an important target.<\/p>\n<p style=\"text-align: justify;\">In his 1874 monograph on mycetoma, Vandyke Carter stated that \u201cfuture investigations will have to determine the precise means to be employed in attaining useful results, other than are arrived at by the use of the knife\u201d.<\/p>\n<p style=\"text-align: justify;\">Mycetoma patients are still waiting.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>Written by Professor Roderick Hay, Dr Kingsley Bampoe Asiedu and Professor Ahmed Fahal (based on the editorial of the same name\u00a0in\u00a0Transactions)-\u00a0 14 Apr 2021 Mycetoma is an important disease that affects the poorest of communities, and which has, for a long time, received little scientific or clinical attention. &nbsp; A recipe for neglect Mycetoma Research [&hellip;]<\/p>\n","protected":false},"author":2,"featured_media":4110,"comment_status":"open","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[25],"tags":[],"class_list":["post-4109","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-blog"],"_links":{"self":[{"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/posts\/4109","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/posts"}],"about":[{"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/types\/post"}],"author":[{"embeddable":true,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/users\/2"}],"replies":[{"embeddable":true,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=%2Fwp%2Fv2%2Fcomments&post=4109"}],"version-history":[{"count":5,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/posts\/4109\/revisions"}],"predecessor-version":[{"id":4171,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/posts\/4109\/revisions\/4171"}],"wp:featuredmedia":[{"embeddable":true,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=\/wp\/v2\/media\/4110"}],"wp:attachment":[{"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=%2Fwp%2Fv2%2Fmedia&parent=4109"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=%2Fwp%2Fv2%2Fcategories&post=4109"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/mycetoma.edu.sd\/index.php?rest_route=%2Fwp%2Fv2%2Ftags&post=4109"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
